We are happy that you have chosen to inquire about Body Wise Physical Therapy, and we look forward to answering any questions you may have.  When you call to schedule your first appointment, we will need the following information:  your name, phone number, email address, date of birth, insurance information, and a brief description of your condition.  After talking with you, we will verify your insurance benefits and call you back to let you know what we found out and to schedule your first appointment.

Upon scheduling your first appointment, we will email you new patient forms, which we ask you to fill out and return to us before you arrive.  These forms provide us with a fuller picture of your condition, your medical history, and any factors that may need to be considered during your treatment.   They also grant us consent to treat and bill your insurance.  Included in these forms is a copy of our HIPAA privacy policies.

What To Expect During Your First Appointment

We recommend that you arrive 10 to 15 minutes early to make sure that all your paperwork is completed.  Please bring your insurance card and a photo ID.  If your physician has written you a prescription for physical therapy, please bring that as well. 

Your first appointment will last between 60 and 90 minutes so that your physical therapist can perform a thorough evaluation of your condition and provide initial treatment if time permits. 

At the end of your first visit, your physical therapist will establish a treatment plan with you and answer any questions you have.  Please feel free to ask any questions as we want you to leave your first appointment with a thorough understanding of the course of treatment planned for you. 

After your first appointment, follow-up visits will be scheduled, and you can plan for these to last about 60 minutes each.  Each visit may include manual therapy, movement-based therapy, education, or progression of a home program to complement the treatment you receive in the clinic.
